Why do I need to show my photo ID?
The Federal Trade Commission issued "Red Flag Rules" that we must comply with to recognize activities that indicate the possible existence of identity theft. Verifying that the patient requesting treatment is consistent with the information provided such as on a driver's license or government issued photo id is one procedure we will be following to identify actions that point to the likely existence of identity theft.
How will we reduce the risk of identity theft?
The Endocrinology Clinic of Minneapolis requires a copy of a picture ID for each patient, such as a driver's license or government issued photo ID. This is used to confirm the patient's identity and is kept in a secure protected environment for clinic use. Clinic staff has been trained to be on the alert for suspicious activity and to take appropriate steps.
Where can I get additional information?
There is additional information in the red binder on the end table in the lobby. If you would like to have a copy ask the receptionist and they will be happy to give you one.
