You must be a patient at our clinic in order to make an appointment with our certified diabetic educators. We provide individual and group diabetes education.

Class Costs: 
The cost for our education programs is covered by most insurance plans. Some insurance plans require a co-payment. Please contact your insurance company to verify if this is a covered benefit under your plan.

Weight Loss Program
We are now offering a 4 part class series followed by two 1:1 visits with a registered dietitian. You will learn tips and strategies to help with weight loss. Topics include portion sizes, label reading, meal planning, dining out, exercise, emotional eating and goal setting. Classes are held on 4 consecutive Thursday’s from 3:00 pm – 4:00 pm in January, March, May, July, September, and November.

We will not bill your insurance for this class series. Payment from you will be required prior to the first class. You can check with your flex plan or HSA to see if you can submit the fee to the plan for reimbursement.

We accept cash, checks, and debit cards. Credit cards from American Express, VISA, Discover and MasterCard.

Diabetic Education Insurance Coverage Form: 
Since coverage varies, for your convenience we have developed a form with additional information and a list of questions to ask your insurance company regarding coverage for diabetic education. We can not guarantee that your insurance will cover your diabetic education. Patients are responsible for balances that the insurance does not pay.
