New Patient Information

Since our physicians only treat endocrine related conditions, our patients must have a primary care physician who can treat any non-endocrine issues that may arise.

We want your initial visit to our clinic to proceed as efficiently as possible.

Please prepare for your visit by doing the following:
If you are a new patient, please download the Patient Registration Form. Complete both pages, read and sign the bottom, and bring it with you to your visit. You will be asked for your insurance card(s), so please bring it along with your co-payment if you have one.

Please download and complete the first two pages of the Patient Medical History Form and bring it with you for your appointment.

Please bring your driver’s license or a government issued photo ID with you to your appointment. This is necessary for you to bring so that we can help reduce the risk of identity theft.

Bring copies of recent lab tests results, x-ray reports, etc., if applicable.

Please bring a list of all your medications to your appointment. If you would like a medication tracking form you can download one from the Minnesota Alliance for Patient Safety website.

Contact the Business Office at 952-927-5316 before your initial visit to confirm insurance coverage, if this was not done at the time your appointment was scheduled.
