Appointments

Appointments are available Monday through Friday from 8:00 am to 3:30 pm. To make an appointment, you can contact us at 952-927-7810. The receptionist will inquire about the nature of your visit so that an appropriate time can be arranged with your physician.

Please call us if you are unable to make your appointment so that the time can be made available to other patients and your appointment can be rescheduled to a more convenient time.

If you do not cancel your appointment within 24 hours of the scheduled date or if you fail to come to your appointment, you may be charged a $25.00 fee.

Please bring a list of all your medications to your appointment. If you would like a medication tracking form you can download one from the Minnesota Alliance for Patient Safety website. If you are a diabetic patient also, please bring your meter and log.
