- Please provide the insurance company name and address, policy and group numbers.
- Present your current insurance card(s) to the receptionist at each visit .
- Notify us immediately of any changes in your insurance coverage.

There are two Networks that we are contracted with:

Minnesota Health Network.

We are providers for many insurance plans including but not limited to:
